Intake form
Questions asked at booking time so you show up to the meeting already prepared.
An intake form collects context before the meeting - what the person wants to discuss, an account number, a link to review. Answers arrive with the booking, so you walk in prepared instead of spending the first ten minutes getting oriented.
For qualification, the same questions can also drive routing - sending the booking to the right person based on the answers.
